Our goal is to provide you with all of our available service
to help you stay at home in comfortable surroundings and assist
you in returning to the highest possible level of functioning.
It is our commitment to assure that none of your rights are
infringed upon as a patient of our agency. If you feel that
your rights have been violated or the agency representatives
have failed to follow policy, you can take the following steps
without fear of reprisal or discrimination. |

RN Team Leader or Administrator of our agency. This procedure
is effective in solving most problems. |

you feel satisfactory action has not been taken, you may contact
the Compliance Officer at 330-929-1050 (mail box #3). If
you would feel more comfortable sending your concerns in
writing to the Compliance Officer at

| Joint Commission
policy forbids accredited organizations from taking retaliatory
actions against patients and or employees for having reported quality of care
concerns to the Joint Commission. |
